I'm baffled by the occurrence of errors being sometimes concealed or ignored, as this practice ultimately benefits no one. From my perspective, errors should not be pinned solely on any one individual, whether they are intentional or unintentional, as they often highlight weaknesses within existing systems. When errors do arise, patients have three fundamental expectations:
Firstly, they expect full disclosure – a clear understanding of what transpired and why.
Secondly, they anticipate concrete steps taken to prevent a recurrence of the same issue.
Lastly, they yearn for a sincere apology.
Embracing these principles and actively listening to our patients can lead to benefits for all parties involved.
